Description:
Researchers at Baker IDI have developed a patient management program whereby nurses visit patients in their homes and make an assessment of their clinical status, management status, and holistic status. This information is used to generate an automated report which identifies the patient’s level of risk and recommends the intensity and frequency of health care intervention required. The nurse and nominated health professionals act to improve treatment plans and lifestyle factors to the level that is required. This tool may enable improved management of patients and allocation of health resources.
